Verification of insurance benefits can be obtained prior to coming in for an initial office visit by contacting the 800 phone number on your insurance card. Benefits are always verified on the patient’s first visit to our facility. Plans will vary depending on agreements your employer has with the insurance carrier. By law we are required to charge all applicable fees related to co-payment, deductibles and co-insurance at the time of the visit.
We are a participating (in network) provider for the following:
- Blue Cross/Blue Shield PPO
- Humana PPO
- Aetna PPO/HMO
- Medicare
We handle injury cases related to:
-NO FAULT/ CAR ACCIDENTS
-WORKERS COMPENSATION/INJURED ON THE JOB
-PERSONAL INJURY
